That turbo has completely seized, extremely common for the restrictor to become gunked on them and restrict flow to them. Inbox me if you would like a quote on repairing it if you haven't so already.

The O ring that seats around the edge will most likely not seal properly and leak boost, or it will catch and cause the CHRA to not sit flush and have the whole assembly to sit on a angle (wheel may touch housings) -

Sounds like the gate has been pulled apart and reassembled! The pinch on the diaphragm is consistent with a failed reassembly! Im assuming it was a brand new unit?The bronzing sounds like it was leaking past the gate and causing your boost issue, OR its a fake gate. Interesting to see how this pans out with your supplier.

Most likely a wastegate/boost issue (low rpms) the wastegate may be inconsistently opening/closing as you hit the threshold. Best bet is to go over vacuum lines to the wastegate, and install a gauge to see whats going on.
