BF XR6 Stalling

I have a 5 speed BF XR6 Ute that likes to stall every now and then. It'll often do it when changing from first to reverse or vice versa. If I give the accel a kick (in neutral) the revs go up, then way down to the point that the car either stalls, or comes very close to stalling.

 

I believe I have checked the O2 sensor and TPS correctly with Torque and a OBD-II Bluetooth Device. The cat is no good but I don't think it's blocked.

 

Have not been able to find any air leaks anywhere. The vehicle still drives very well.

Inspected the PCV system today, the valve itself and the asssociated hoses all seem fine. No problems found.

 

Apparently the inlet manifold gasket is prone to failing. I may try spraying something flammable around that area.

I should add that sometimes the car acts as though the fuel has been cut and I have to give the accel a bit of a kick to get the car accelerating again. It's very intermittent and I have never had a chance to look at the TPS reading when it happens. 13.7% to 78% it read I remember correctly. Does that sound right, and could more acceleration possibly be obtained by somehow increasing the TPS to output 100%?
