The contractor had isolated thepower and was disassembling the hoist post to accessthe ram. Realising that the hoist arm was too low toreach the ram the contractor re-engaged the power tolift the arm to access the ram. The contractor finishedwork for the day failing to isolate the power. Thefollowing morning the contractor returned to theworkshop to complete the repair work on the ram andproceeded to assemble the hoist post without isolatingthe power again. On reaching the top of hisaluminium ladder he apparently leant on the exposedwires from the hoist arm. The wires made contactwith the ladder and the contractor received an electricshock (415v). f*ckING PAPERWORK!!!
