Here is a test report. Any ideas?

I don't know much but it sounds like you've got a stuffed o2 sensor. in tuned cars they generally turn the second one off so I think you only need to replace one of them (the one that's easier to access iirc)

 

If you wipe your finger on the rear exhaust is it covered in black stuff? does it blow a puff of smoke when you accelerate?

+1 for a new sensors.  At least give the ECU the best chance of seeing what's actually going on for a short time there. If you've got the ability to log, keep an eye on the trims and you might notice if its rich across the board or maybe just at certain load points

Two sensors, one before one after.  Believe the front is more for O2 composition & the rear for temp measure vs the front.  

 

If you are only getting a code when it's cold, it also sounds like the cold temp compensation could have been modified.  Is there a reason you haven't taken it back to the tuner yet to have them investigate this?
